for success in TB treatment

The stairs leading up to the apartment of the Singh family in the center of the town of Jalandhar, India, are steep and outworn. Breathless, one arrives at the first floor only to discover another winding flight to climb. Yet, 14-year-old Suchi Singh does it with determination.

She climbs it although it makes her breathing heavier than before. She climbs it although she can longer hear the echo of her heels as she used to. She climbs it although her young body is over-run by a disease forcing her into a difficult treatment.

As she comes to greet me, her playful dark eyes sparkle behind the handkerchief covering her nose and mouth. Except for the beige piece of cloth, nothing speaks of her suffering from multidrug-resistant Tuberculosis (MDR TB), a strain which takes two years to cure and is 200 times more expensive than the treatment for category I patients. Along with painful injections, and the unwelcome side effect of losing part of her hearing, Suchi is taking seven pills every day.

“It’s a lot of bitter pills to swallow, but I wish to get well,” she says, now halfway to the cure. Two of the reasons for the successful adherence to her treatment stand next to her, the Red Cross volunteers Charanjit and Hardeep. Since Suchi was diagnosed with MDR, they have made constant house calls, provided advice, supported her with transport to the hospital and added on supplementary protein to her diet, as weight loss is frequent among TB patients.

“To submit to the toxic side effects of the MDR treatment is a challenge to anyone. The human touch in the TB intervention is the key to success,” says Dr. Kailash Raizada, Senior Advisor to the International Federation of Red Cross and Red Crescent Societies (IFRC). With nearly two million new cases in India per year, the task to combat TB in India is monumental. According to India’s Revised National TB Control Programme, about 800,000 cases are infectious and causing 1,000 deaths every day.

Since 2009, the IFRC has been assisting the India Red Cross Society in an intervention, which includes 1,180 patients who are among the most vulnerable people living with TB, in particular category II patients. The term category II implies someone who has failed previous TB treatment, relapsed after treatment, or defaulted during previous treatment.

The scope of the intervention, which covers seven states, is especially aimed at these patients, as they run the risk of developing MDR TB. Nonetheless, it takes persistence and perseverance to ensure the 100 percent adherence to treatment. Navdeep Kaur, District Coordinator of the TB Programme in Jalandhar, knows. One of her patients was Gurprett Singh. One day, he simply disappeared. “His parents were quiet about his whereabouts. We returned for several days, but not a sight of him. Asking around in the community, we finally found him in jail,” Navdeep Kaur recalls. Alerting the prison authorities about his medical condition, Gurprett Singh was diagnosed and put under treatment again. With further support from the Red Cross, assisting with nutrition in jail, he then followed through with the treatment and is now cured.

India Red Cross is now looking into ways of expanding the TB programme for category II patients. “Extending our humanitarian services to reach more category II patients will be a contribution to making India a TB free country. Every patient we cure means less human suffering and less money spent on treatment. Also, an infected person risks spreading the infection to fifteen people, which we hinder through our intervention,” says Dr. SP Agarwal, Secretary General of India Red Cross Society.

In her paper, "Sri Lanka: Unrestricted Warfare and Limited Protective Humanitarian Action," author Norah Niland addresses the protection dimension of humanitarian action in the Sri Lankan Civil War. Niland argues that the failure of the intergovernmental crisis management and the human rights machinery was exacerbated by the relief system's lack of agency safeguarding humanitarian space and the protected status of civilians. The paper is followed by critical comments by Sir John Holmes, Former UN Under-Secretary General and Emergency Relief Coordinator, Head of the UN Office for the Coordination of Humanitarian Affairs, and Miriam Bradley, Postdoctoral Researcher, Program for the Study of International Governance, the Graduate Institute, Geneva.

A few of the world's leading experts in the global health field gathered last Wednesday to answer some of the most relevant questions for those seeking careers in the global health sector. IIHA intern Evelyn Lopez listened in on the webinar, “Global Health University Webinar: Careers in Global Health, Advice From The Experts," and compiled a list of the Top 10 Lessons for Success in Global Health Careers.

International Humanitarian Affairs Minor: The IIHA has established the International Humanitarian Affairs Minor, available to undergraduate students at Fordham College Lincoln Center and Rose Hill. This interdisciplinary minor offers students a comprehensive understanding of issues central to contemporary humanitarian affairs. The Minor offers an in-depth analysis of the humanitarian field, enabling students to acquire theoretical knowledge and practical experience.

International Humanitarian Studies Major: In considering international responses to suffering, undergraduates in this major will draw upon a wide array of intellectual tools and academic frameworks. Ethical, moral, cultural and spiritual considerations confront logistic, economic, scientific and geo- political modes of thought in circumstances of vast complexity: violent conflict (including gender-based violence), famine and natural disaster. The major will also contribute to central aims of Fordham’s mission: research and education that assist in the alleviation of poverty, the promotion of justice and the protection of human rights as part of a cosmopolitan education that prepares students for leadership in a global society.

Masters in International Humanitarian Action (MIHA): The IIHA and the Graduate School of Arts and Sciences (GSAS) have joined together to offer a comprehensive Master’s program that combines the experience of long-time humanitarian professionals with the academic strength of Fordham faculty. The program combines the experience of seasoned humanitarian professionals with the academic strength of Fordham faculty from the School of Law and the Graduate Schools of Business and Social Service, who provide students with the political, historical, legal and ethical contexts of past, present and future emergencies.

The Institute of International Humanitarian Affairs (IIHA) at Fordham University, working with external humanitarian partners, offers programs throughout the world at both the graduate and undergraduate level. Through these programs, IIHA trains students for careers in the humanitarian field by combining an academic approach with the shared practical experience of faculty, external speakers and students.
